On August 18, 2011, a Proton-M/Breeze-M launch vehicle lifted off with Express-AM4 satellite from Baikonur. The upper stage failed to insert the satellite into the proper orbit. Express-AM4 was to provide telecommunications and broadcast services to Russia and neighboring states.
